SUPPORTING MEMBERS — INDUSTRIAL COMMITTEE

Registration is now possible and we are cordially inviting you to submit a membership declaration form to become a supporting member of the Polish Society for Probiotics and Prebiotics! Supporting members receive a diploma at the beginning of every new calendar year. Precise, step by step information how to become a supporting member of the Society may be found below.

A. The membership fees are as follows:

• for supporting members, the annual fee is PLN 2000.00 (i.e. two thousand Polish zloty) (approx. €525 depending on the exchange rate)*
  

* The membership fees are due to be paid until the end of the first quarter of the calendar year via bank transfer to the given account; and in case of a member being accepted later on in the calendar year - the fees are due 30 days after a person is accepted as a member of the Society. Please visit the National Bank of Poland website for current exchange rates.

B. To become a member, please follow the procedure:

1. Click on the text or icon in point A. to open a membership declaration form.
2. Print the form out (two A4-size pages).
3. Fill the form in, then sign it in the appropriate place.
4. Send the completed application form to the head office of the Society:
   Polish Society for Probiotics and Prebiotics

   Poland
   
5. Wait for the decision of the Board of Directors, which will be sent to the e-mail address on the declaration form (or via mail in case the applicant has no e-mail).
6. Only after approval of the membership by the Board of Directors, the member will have to pay the membership fee, as specified in point A., using the bank details below.

7. After the membership fee is paid, the name of the company in text format or the name and surname in case of a private/natural person will be placed under the heading "Supporting members in the year ...:" in the section above.
   

C. Who may become a supporting member of the Society?

Supporting member may be a private person or business interested in the activities of the Society and willing to support the Society financially or in other material way, that will be accepted by the Board of Directors on the basis of a written declaration.



D. Supporting member has the right to:

a) Participate in general assemblies with an advisory vote (in case of legal entities/business - through representatives).
b) Make use of the rights described in points c, d, f of Article 14 (§ 14.) of the original Bylaws of the Polish Society for Probiotics and Prebiotics (original name: Statut Polskiego Towarzystwa Probiotycznego i Prebiotycznego) and in case of a private/natural person also make use of point e of Article 14 (§ 14.) of the Bylaws. Legal entities/business may only receive a membership diploma from the Society.

Supporting members may organize themselves into the so called Industrial Advisory Committee, in order to support the activities of the Society according to Bylaws.
Such an Industrial Advisory Committee functions according to rules and regulations accepted by the Board of Directors.


Quoted here is Article 14. (§ 14.) of the Society's Bylaws:

"Ordinary member has the right to:
a) participate in general assemblies with a decisive vote,
b) actively and passively elect the authorities of the Society,
c) participate in the actions of the Society according to the rules and regulations of its authorities,
d) make use of all the apparatus of the Society according to the set rules,
e) wear the Society's logo,
f) raise postualtes and put motions forward to the authorities of the Society."



E. Supporting member is obliged to:

a) abide by the Society's bylaws, rules and regulations of authorities of the Society,
b) actively participate in the fulfilment of the aims found in the bylaws of the Society,
c) regularly pay member's fee and other fees and amounts due, as described in the bylaws of the Society as well as in the rules and regulations decided upon by the authorities of the Society.
